One row per project. Columns: LOCATION, TYPE, PROJECT, then one column per criterion (C1 … C12, or your own names).
Optional weight row: put WEIGHT in the Location, Type or Project cell and the weights under each criterion. Without it, the current weights are kept when the criteria names match.
Commas, semicolons or tabs all work, as do decimal commas and UTF-8 or UTF-16 files. A CSV saved straight from the MCA sheet (File › Save As › CSV UTF-8) is recognised too. Excel writes numbers as they are displayed, so show enough decimals before saving.
